Documents needed

Completed application form DS-11, signed and dated in front of Acceptance Agent(DO NOT SIGN UNTIL YOU ARE INSTRUCTED TO DO SO) 

2 identical passport photos-size 2"x2", in color and taken within the last six months.

A picture ID in the form of a valid drivers license, current school, work or military ID.

One of the following:

     State(not hospital) Certified birth certificate (Original's must go with application)

     Original Naturalization or Citizenship Certificates. 

     Your most recent US passport current or expired.
